Position Summary
The Clinical Nurse Navigator/Coordinator functions as a member of the multidisciplinary team who has clinical nursing expertise in an assigned area. They focus on the clinical and educational aspects of care, acting as a clinical resource, advocate, and educator. They are present in the clinic and serve as a vital member of the care team.
Essential Duties and Responsibilities
- Demonstrate a strong commitment to patient-centered care within an interdisciplinary environment.
- Follow up with patients on testing and procedure results in coordination with providers.
- Prepare clinic prep notes for new patients and returning patients, which includes scanning notes/imaging, contacting referring physicians or imaging groups, and contacting patients on any issues or items needed for visit.
- Assist with coordination of follow-up visits, orders, and referrals.
- Initiate/Participate in quality improvement, patient safety, and patient experience initiatives.
- Participate in training and education of support staff.
Minimum Qualifications
Education
- An Associate's degree or higher in Nursing, or an equivalent combination of training and experience.
Licensure
- A current and valid D.C. license as a Registered Nurse or evidence of application for licensure in D.C.
Experience
- A minimum of 2 years of experience as a Registered Nurse, OB/GYN experience preferred.
Physical Requirements
- Sit, walk, reach, bend, or twist for long periods of time in a clinical setting.
- Must be able to lift, carry, push, or pull up to 100 lbs. as part of the role.
- Regularly exposed to healthcare settings that may require personal protective equipment.
- Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment.
