Join University of Chicago Medicine as a Clinical Pharmacist in the Meds2Beds Pharmacy.
In this role as a Clinical Pharmacist, you will assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, you will provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacists adeptly function in pharmacy operational areas, including but not limited to central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas.
Job Summary
Responsible and accountable for the provision of safe and effective drug therapy and other pharmaceutical care services intended to achieve outcomes related to the cure, diagnosis or prevention of disease, elimination or reduction of patient’s symptoms, or arresting or slowing of a disease process, and improve a patient’s quality of life in collaboration with a multi-disciplinary team. The pharmacist also assumes the responsibility for pharmacy technicians that work directly under his/her supervision by ensuring the proper preparation of drug products and managing the workflow.
Essential Job Functions
- Maintains a standard of performance that is in compliance with JCAHO, ASHP practice standards/guidelines, federal/state licensing acts, and UCH and Pharmaceutical Services Policies and Procedures.
- Using a multidisciplinary approach, ensures patient is discharged with appropriate therapy.
- Provides patient counseling to ensure patient understanding of medications before leaving the hospital.
- Interprets, evaluates, and accurately dispenses all drug orders in the patient’s best interest as assessed by appropriateness of drug selection, contraindications, dose, side effects, drug interactions, food interactions, duration of therapy, route of administration, and cost.
- Identifies, resolves and prevents potential and actual drug-related problems. Recommends rational therapeutic alternatives as needed to prevent future problems.
- Accurately checks medication orders that have been entered into the pharmacy computer system by the pharmacy technician, ensuring that patient allergies, drug duplication, and drug interactions do not exist which contraindicate therapy and that the drug dose is correct. Enter orders directly into computer system if necessary.
- Supervises the quality/quantity of work and work performance of the pharmacy technician.
- Coordinates pharmacist and technician coverage of the area of responsibility during meal breaks or in-services. Assists other staff members in other areas during high workload.
- Supervises pharmacy technicians working under their direction to provide optimal pharmaceutical care and services.
- Participates in the orientation, education, and mentoring of new staff (i.e., pharmacists, nurses, pharmacy technicians, etc.) as needed.
- Serves as preceptor to pharmacy students and PGY1 residents.
- Participates in the preparation of articles for publication in the “Topics in Drug Therapy” or other UCH newsletters as requested.
- Performs other duties as requested by senior management.
Required Qualifications
- Graduate of an ACPE accredited College of Pharmacy.
- Doctor of Pharmacy or other advanced degree highly preferred.
- Eligible for licensure as a pharmacist in the state of Illinois (complete licensure process within 60 days of employment).
- Completion of an ASHP accredited PGY-1 Pharmacy Residency required OR BCPS/BCACP Certification and at least 2 years of relevant experience.
Preferred Qualifications
- Advanced Degree, Master of Science, MBA, or Doctor of Pharmacy highly preferred.
- Extensive clinical experience and development may be considered in lieu of formal training.
