Position Overview:The Bereavement Coordinator completes Bereavement Assessments, establishes Bereavement Plans of Care and provides/arranges Bereavement contacts according to the Plan of Care.
Essential Job Functions:
- Complete Bereavement Assessment as per policy timeline.
- Ensure initial bereavement contact made by staff (attend wake and/or funeral)
- Establish Bereavement Plan of Care appropriate to needs of family members identified in Bereavement Assessment.
- Coordinator/schedule bereavement contacts per Bereavement Plan of Care and Assessment: visits, phone calls, letters, cards, bereavement material/literature, memorial services, and other bereavement activities.
- Establish Bereavement chart
- Document all Bereavement contacts and file all contacts in Bereavement chart
Maintain, update, and review Bereavement Chart for compliance with Bereavement Plan of Care. - Continue to monitor family member bereavement needs and coping skills. Determine if need exist for outside grief counseling or other resources.

Requirements:
- Desired Bachelor of Science degree related field such as Social Work or Counseling or minimum of 1 year of Hospice Chaplin experience can substitute for a degree.
- Prior training or experience regarding loss, grief, and dealing with patients and families in an emotionally changed environment required.
- Must have strong communication skills.
- Must be able to relate positively and favorably to patients, families, visitors, customers, physicians, and work cooperatively with other employees.
- Must be able to speak, write, read and understand English
Occasional lifting, carrying, pushing and pulling of 25 pounds. - Must be able to lift 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Must be able to sit and climb stairs.
- Must have visual and hearing acuity.
- Must have strong sense of smell and touch.
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
- Maintain an orderly, confidential, and safe work environment.
- Practice universal precautions at all time.
Preferred:
- Desired one (1) year related experience at a level necessary to accomplish the job.
- Prior experience with Electronic Medical Record systems preferred.
- Humanitarian attitude and good understanding of hospice philosophy
