Seeking an RN Case Manager to care for hospice patients. We are looking for a hospice nurse to provide care and support to terminally ill patients. Your services should bring comfort to the person who faces the end of their lives. Haydel Memorial Hospice is the only locally owned and operated hospice agency in this area. Our goal is to provide the highest standard of care giving our patients dignity, quality and comfort in their final stages of life.

Responsibilities

Performs physical and psychosocial assessment on all hospice patients.
Initiates plan of care, reevaluates, and updates as necessary.
Supports and maintains a culture of safety and quality.

Qualifications

Current Registered Nurse Licensure in the State of Louisiana
Demonstrates leadership with the ability to relate well with the staff
