Description

Position Summary
The Charge RN is responsible to maintain the nursing philosophy, objectives, and standards of quality nursing care; and ensure compliance with standards, codes, rules and regulations, and the policies set-forth by Woodland Pond.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Administrative Functions:
1. Notifies the Director of Nursing and Administrator on Call as required.
2. Participates actively with IDT and follows-through on actions determined by the Team.
3. Is responsible for every resident in the Health Center.
4. Serves as a resident advocate and ensures resident rights are maintained.
5. Supports the medical staff.
6. Works collaboratively with all departments at Woodland Pond.
7. Works with staff in the planning of the units services, programs, and activities.
8. Ensures consistency between all units in nursing care.
9. Ensures staff comply with policies and standards of care.
10. Ensures completeness of Accident and Incident reports.
11. Serves as a leader and resource for nursing staff.
12. Participates in investigations as assigned.
13. Ensures safe quality care is provided to the residents daily.
14. Is a liaison with residents, families, and other departments to adequately plan for and provide resident care.
15. Guides LPNs and CNAs responsibilities.
16. Ensures accurate, appropriate, and complete documentation by all nursing staff.
17. Ensures accuracy of Care Plan, Kardex, MDS, and Assessment.
18. Completes timely as assigned.
19. Effectively communicates the plan of care.
20. Ensures that resident care plan is followed.
Communication:
1. Ensures appropriate and ongoing communication to the resident, family, staff, MD, IDT, etc.
2. Uses communication to enhance quality of resident care and staff working environment.
Personnel/Staff Development:
1. Assists with employee evaluations, employee praise, and discipline as necessary.
2. Actively participates with staff education.
3. Promotes an environment of learning.
4. Ensures staff attends mandatory in-services and meets minimum continuing education requirements.
5. Conducts routine staff audits to ensure care is being delivered per care plan and medical staff orders.
6. Ensures work consistency among staff.
7. Promotes a positive, friendly work environment.
Equipment and Supplies:
1. Ensures appropriate supplies are available and monitors that supplies are utilized appropriately to avoid waste.
2. Is cost-effective.
Policy and Procedure Manuals:
1. Ensures staff awareness of manuals, including other departments.
2. Uses manual and assists other staff on using policies as necessary.
Quality Assurance:
1. Constantly strives to enhance the resident and staff experience.
2. Reports and completes necessary paperwork for each resident incident/accident/change in condition.
Safety/Infection Control:
1. Ensures staff compliance with standards and regulations.
2. Reports potential outbreaks and takes appropriate measures to control/eliminate infection control exposures.
Staffing Coverage:
1. Required to ensure appropriate coverage for the nursing department 24/7, including weekends, holidays, and off-shifts.
2. Works beyond normal working hours, or in other positions temporarily when necessary.
3. Is subject to callback during emergency conditions.
4. Takes RN on-call as needed.

2. Encouraged to attend out-of-facility continuing education seminars.
3. Completes all required training and in-services.
4. All other duties as assigned.

Qualifications

Maintains predictable and reliable attendance.

Qualifications

RN graduate of an accredited school of nursing.
Minimum two years of clinical nursing experience preferred.
Holds a current, unencumbered RN license to practice in the State of New York.
Proficiency with computers and standard office software.
Valid CPR certification.
