Next Generation MIHP is a program of Because of His Love Outreach (BOHL), a non-profit organization based in Detroit.

We are committed to loving every person, bringing hope to every situation, and empowering the next generation. BOHL provides supportive, educational services to pregnant women, infants, and their families, including support groups and access to essential baby items.

Duties:

-At least ONE year MIHP experience or hospital experience working with pregnant women and infants.

- MUST BE WILLING TO TRAVEL PRIMARILY IN THE DETROIT AREA.

- MUST BE COMFORTABLE WITH HOME VISITING.

- MUST BE COMPETENT AND ABLE TO PROFESSIONALLY WRITE AND VERBALLY COMMUNICATE

- Please do not apply if you are looking for a quick side job. These families need a dedicated case manager who is compassionate and has the time and ability to provide quality care.

- Pay is per 30-minute client visit and successful completion of clinical progress notes.

- Provide case management to Medicaid clients; pregnant women and infants under the age of 12 months with Medicaid in the Metro Detroit, Wayne County area.

-Conduct developmental screenings (ASQ'S) for infants every 4 months or as needed

-The ability to engage with and empower clients

-A self-starter, organized, and the ability to work independently with little direction

-A Registered Nurse (RN) or Social Worker (LLBSW, LBSW, LLMSW, LMSW) with an active license to practice in the State of Michigan.

-The ability to utilize an Electronic Medical Record (EMR) System

-Conduct client revisits once per month for at least 30 minutes, and complete progress notes within 24 hours of the visit.

-The ability to provide resources to clients based on their needs.

-RN's are NOT required to provide any medical interventions. This role is basic education to empower clients. Referrals to the clients health provider should be made. We do not provide medical advice.

-Social Workers are NOT required to provide any counseling. This role is basic education to empower clients. We do not provide one on one counseling.

-Pay is based on per visit: $30 for telehealth - 30 minutes, $40 for home revisits - 30 minutes, $50 for home health assessments - 60 minutes

-This position is a contract 1099 position
