Position Description

Provide medical nutrition therapy and diabetes care and education to patients in the primary care setting in a Federally Qualified Health Care Clinic. Provide education on nutrition and diabetes topics in the individual and group setting, adapting approach to patient's learning needs and abilities. Document interventions and progress in the EMR system. Promote health and wellness by providing education to staff and community on nutrition and chronic disease management.

Minimum Qualifications

Registered Dietitian with the Commission on Dietetic Registration.
Certified Dietitian with the State of Vermont.
Certified Diabetes Care and Education Specialist (CDCES), or CDCES eligible.

Desired Qualifications

Master’s level education preferred.
Experience in clinical use and management of diabetes care technology including insulin pumps and continuous glucose sensors.
Excellent interpersonal and teamwork skills.
Remains updated on current nutritional and diabetes care research.
