Overview:
The Transplant Coordinator is responsible for the coordination of clinical care for renal transplant patients in the outpatient clinic setting at Legacy Good Samaritan Hospital and Medical Center. The Transplant Coordinator works within an interdisciplinary team to provide education, monitoring, advice and care for transplant patients and their care partners.
Responsibilities:
Coordinates clinical care including patient screening, patient assessment, phone call triage, case management and follow-up for renal transplantation.
Follows program standards to meet Centers for Medicare and Medicaid Services (CMS) and United Network for Organ Sharing (UNOS) guidelines.
Communicates effectively with interdisciplinary team, referring providers and ancillary departments.
Provides appropriate education related to transplant and living donation, and documents education in medical record.
Organizes admission of transplant patients and donors if appropriate.
Reviews and records labs, vital signs, medications, and input/output as appropriate
Recognizes and communicates abnormal lab and test results to medical team as appropriate.
Assists with patient clinic flow, manages follow up appointments, lab draws, outpatient procedures and imaging.
Qualifications:
Education:
Academic degree in nursing (BSN or MSN/MN) required; MSN/MN preferred. Internal candidates may be considered with verification of a BSN or MSN/MN in progress
Experience:
Minimum of five years work experience related to area of responsibility required.
Knowledge of regulatory requirements and relationships with related organizations including Centers for Medicare & Medicaid Services (CMS) and United Network for Organ Sharing (UNOS) required.
Management experience or experience in developing kidney transplant programs preferred.
Skills:
Thorough knowledge of operations, including staffing and scheduling, budget management, and workflow planning.
Ability to manage a broad span of control through implementation of a self-directed team approach.
Strong communication and leadership skills, and a willingness to lead by example.

Licensure:
Current applicable state Nursing License.
Pay Range: USD $47.95 - USD $71.45 /Hr. Working Conditions: Must be able to lift push pull 0 20 pounds. Voluntary use of N95 permitted May not enter isolation areas or provide COVID 19 care Annual E required .
