Job Title: Supervisor - Case Management
Job Type: 6-Month Contract
Location: Detroit, MI 48202
Shift: Days | 8 AM - 4:30 PM, M-F
Schedule: 5 x 8-hour shifts
Job Summary:
The Supervisor of Case Management at Henry Ford Health (HFH) oversees daily operations of case management, ensuring effective care coordination and optimal patient outcomes. This role involves guiding a team of professionals in delivering comprehensive healthcare solutions and participating in policy development and performance improvement.
Key Responsibilities:
- Manage daily case management activities in a specific unit.
- Evaluate staff performance and provide feedback for professional development.
- Serve as a resource for case managers addressing complex patient needs.
- Collaborate with leadership to implement best practices and develop educational materials.
- Lead performance improvement initiatives and staff training programs.
Minimum Requirements:
- Education:
- Registered Nurse (RN) or a Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
- Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC) preferred.
- Experience:
- Case management: 2 years (Required)
- License/Certification:
- RN License (Required)
- BLS Certification (Required)
