Position Summary:
The Hospice Intake RN is an integral part of the intake and admission process and is responsible for managing the eligibility aspect of the referral process for patients entering hospice care, ensuring a compassionate and seamless transition for patients and their loved ones. As such, this person must understand the Medicare Hospice Benefit. The Hospice Intake RN coordinates with multiple other team members via text messaging, phone calls and/or email and conveys pertinent information in a timely manner. Accurate maintenance of the electronic health record is a fundamental aspect of this position. This role requires a balance of strong clinical skills, communication abilities, and compassion to support patients during their end-of-life care decisions.

Qualifications:
- Graduate of accredited school of nursing
- Current Minnesota RN licensure
- Two years recent experience in hospice
- Critical thinker
- Working knowledge of computer hardware and software
- Ability to read, write, speak and comprehend English
- Excellent oral and written communication skills
- Ability to prioritize and organize multiple tasks
- Ability to work independently
- Excellent interpersonal skills
- Ability to respect confidentiality
Job Duties:
- Reviews patient medical records to assess for hospice eligibility based on Local Coverage Determinations (LCDs)
- Gather and review additional clinical documentation as needed to determine hospice eligibility
- Accurately enters information into the electronic health record in real time
- Collaborates with Medical Director to determine eligibility
- Facilitate communication between providers, patients, families, and hospice care teams
- Communicates findings to Admission RN, utilizing admission template
- Education patients and families on hospice philosophy, services, and care options
- Maintains compliance with regulatory requirements and hospice policies
