Location: Multiple Locations - traveling to clients (MILEAGE REIMBURSEMENT INCLUDED) & TELEHEALTH
Schedule: Full Time: Monday - Friday; with OPTIONAL on-call
Compensation: $120-145K
Additional Compensation: $8-12K (approximate, additional pay comes in for after-hours calls, mileage, etc.)

Nurse Practitioners are key members of Lifespark Health's professional medical practice. We deliver primary care and consultative services to frail and complex patient populations. This currently includes home-based primary care in the community and on senior housing campuses, attending physician and APRN services in transitional care units, and in our innovative Hospital at Home program. We utilize a combination of in-person and telehealth visits inside a multidisciplinary team-based model. Patients and their caregivers receive expert, accessible, and compassionate care.
This collaborative and dynamic medical practice is led by industry experts in geriatrics and senior care services. It is delivered under the umbrella of a larger Lifespark enterprise which provides whole person senior care to broad populations of seniors. Lifespark has value-based arrangements with payers which allows clinicians to practice independent of traditional healthcare and payment constraints. These payment methodologies allow clinicians to practice the way they always envisioned when they chose to join the caring profession. And you will be part of a team that focuses on seniors who need you the most!
Qualifications:
- Current Minnesota APRN or PA license with current board certification
- Current DEA license
- Certification in gerontology/geriatrics preferred, or strong experience in primary care, community based, and/or palliative medicine
- Meet threshold criteria/qualifications for Credentialing and Privileges.
- Communicates effectively in verbal, written, and electronic methods.
- Able to work in non-traditional settings (client homes, nursing homes, and from home/Telehealth)
