Health Educator to Join our North Campus location. This position will be responsible for participating in delivery of client education and disease management interventions and for performing health coaching for clients, across multiple lines, for health improvement/management programs for chronic diseases; maintaining timely and thorough documentation of all services utilizing agency, reimbursement, regulatory and professional standards of practice. Work is performed under the administrative supervision of the Nurse Manager at North Campus.
Essential Functions Conducts behavioral health or medical assessments to identify individual client knowledge, skills, medical and behavioral needs.
Identifies, develops and/coordinates specific health coaching plan needs to address objective and goals identified during assessments.
Collaborates with other members of the health care team as well as external providers to facilitate implementation and monitoring of coaching plans.
Completes medically related assessments including mental status exams, medication side effects, monitoring adherence, administering treatment scales, i.e. the PHQ9 and taking vital signs on all clients per visit.
Facilitates educational groups at the agency and one-to-one educational sessions with individuals in their home as needed.
Lab value management to include venipuncture and coordination with Medical and Psychiatric providers to obtain data that will drive treatment and coaching plans.
Collaborate with clients to establish health improvement plans, set personalized goals, support customers in achieving those goals and evaluating the outcomes of the coaching plans.
Monitors and evaluates effectiveness of interventions and/or health coaching plans and modify as appropriate.
Facilitate referrals to health professionals for behavioral health and medical needs.
Use motivational interviewing to empower clients to facilitate health behavior change.
Provide linkage to community resources to promote wellness.
Refers clients to catastrophic case management.
Maintains thorough documentation of daily activities including nursing and treatment services.
Attends and participates on integrated treatment team; attends staff meetings and trainings as required. Collaborates with primary care, mental health, developmental disabilities services and substance use services as clinically indicated.
Coordinates schedule with providers and front desk staff.
Provides crisis intervention services as needed
Maintains productivity standards as set by division.
Implement and utilize collaborative documentation.
Provides orientation to newly admitted clients at North Campus
Facilitates new hire orientation at the North Campus
Performs staff nurse duties at RICH clinic and North Campus as needed
On-call rotation as scheduled
Performs other duties as assigned.
Position Requirements
Education and Experience - Associates degree or higher with coursework in nursing and at least one year of experience working with individuals with serious mental illnesses.
Special Requirements - Current license as a Registered Nurse issued by the Commonwealth of Virginia.
- CPR certification.
- Valid driver's license in the Commonwealth of Virginia.
- Computer proficiency
Qualifying RN staff will be eligible for $4,000.00 sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy.
