Sentara - Oncology Service Line is currently hiring an Oncology Nurse Navigator (RN), who will primarily support hematology-oncology and gynecologic oncology patients. This is a Full-Time position with day shift hours and great benefits!An Oncology Nurse Navigator (RN) supports cancer patients. Responsible for providing patient and family education regarding findings, diagnosis, workup, lifestyle recommendations, and treatment plans. Assessing psychosocial needs and assisting with barrier resolution. Responsible for approximately 250 patients per year. This position is integral in improved patient satisfaction, staff satisfaction, provider satisfaction, reduced length of stay, reduced ED visits and readmissions, and quaternary growth.
The primary location will be at Sentara Norfolk General Hospital with the ability to travel if needed to meet patient care and service line needs in the Hampton Roads region, including Northeastern North Carolina.
Primary duties include but are not limited to:
• Facilitating discussions and communication between patient, caregiver, and care team regarding diagnosis, treatment, and symptom management
• Assessing client needs and barriers to care through the continuum of care.
• Providing support, education on medications, treatments, and resources available internally within Sentara and externally in the community and nation.
• Facilitating referrals to appropriate resources.
• Leading, coordinating, and participating in various quality initiatives related to Oncology Service Line such as multidisciplinary conferences, tumor boards, etc.
Position has flexibility with hours. The department is open from 6:30am - 6:00pm.
Minimum Requirements
Experience: 3+ years of clinical nursing experience required.
Education: RN-Associate's Degree OR RN-Bachelor's Level Degree OR RN-Diploma (Nondegree) OR RN-Master's Level Degree
BLS required within 90 days of hire and maintained thereafter (if in clinical setting)
Specialty certification (OCN, CBCN) and/or completion of patient navigation training based on specialty area required within one year of eligibility
Prior work experience with oncology patients and/or case management is strongly preferred.

Job SummaryResponsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Functions in one of the following practice settings: Acute Care, Service Lines, Ambulatory/Community-based, Home Health, and Long Term Care.
BLS required within 90 days of hire and maintained thereafter (if in clinical setting)
For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC or CGMT-BC). For other service lines, certification based on specialty area required within one year of eligibility.
3 years Oncology and Case Management experience preferred.
