Nurse Practitioner - Family Practice

Compensation: $100,000.00 - $130,000.00
Essential Duties and Responsibilities:
- Obtain detailed medical histories and conduct thorough physical examinations to evaluate the health and wellness of patients.
- Order, perform, and interpret diagnostic tests such as lab work and x-rays.
- Develop individualized care plans based on patient assessments and diagnosis, ensuring seamless integration of treatment strategies.
- Collaborate with physicians and other healthcare professionals to discuss treatment plans and patient progress.
- Administer medications and injections as part of the management of acute and chronic conditions.
- Provide patient and family education focused on lifestyle changes, disease prevention, and management.
- Maintain patient records with meticulous accuracy, as required by regulatory bodies and medical law.
- Participate actively in team meetings and professional development sessions to go beyond conventional care delivery.
- Manage a personal panel of patients, fostering long-term health relationships through continuous care standards.
- Cover occasional extra shifts including early mornings or evenings and Saturday mornings as required.
- Work with the latest medical technologies like the Epic EMR system to streamline patient management processes.
Minimum Position Qualifications:
- Current and valid New York State license/registration as a Nurse Practitioner.
- Specialty area of certification consistent with family practice.
- NPI number and either a current Medicare Identification Number (PTAN) or eligibility to obtain one within 6 months of start date.
- Current CPR certification either through the American Heart Association or the American Red Cross.
- Compliance with all applicable state and federal laws and regulations pertaining to family practice and nurse practitioners.
Preferred Qualifications:
- 3-5 years of previous experience in primary care or outpatient settings.
- Demonstrated ability in using EMR systems, particularly Epic.
- Exceptional ability to communicate effectively, both verbally and in writing, with patients, families, and other health professionals.
- Proven track record of responsiveness and positive outcomes in patient care management.
- Capacity for leadership in a clinical setting and potential for professional growth and development.
