Practice Nurse - Nurse Practitioner Primary Care
Summary: The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs mission. The NP provides advanced clinical practice, consultation, and management and has prescriptive authority for a panel of patients or program as indicated by their position.
The incumbent is accountable to the Chief of Staff.
Responsibilities: The primary purpose of the nurse practitioner (NP) is to practice to the full extent of their education, training and certification as a licensed independent practitioner to provide healthcare to individuals, families, and/or groups in a variety of settings to include virtual modalities. The NP functions as the case manager for a complex Veteran population, providing hands-on and virtual patient care, and assumes primary medical responsibility for the patients assigned to their PACT team. Major duties and responsibilities of the position include but are not limited to:
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
- Orders and interprets labs, tests, and diagnostic studies.
- Provides timely follow-up on results, including completion of view alerts.
- Performs and records history and physical examinations and records data from clinic visits.
- Enters progress notes in computerized medical record and completes workload documentation in a timely manner.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals and consults to specialty services.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
- Consults other specialties as appropriate, according to service agreements.

Requirements: U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA.
English Language Proficiency. In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Selective Service Registration is required for males born after 12/31/1959. You may be required to serve a probationary period. Subject to background/security investigation.
Selected applicants will be required to complete an online onboarding process. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP).
Qualifications: Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency.
License and Certification: All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
Preferred: Experience within the adult primary care outpatient setting within the last two years. Women's Health (WH) experience preferred but not required.
Physical Requirements: This position requires heavy lifting, 45 pounds and over, heavy carrying, 45 pounds and over, straight pulling 1 hour, pushing 1 hour, reaching above shoulders, use of fingers, both hands, walking and standing 7 hours, kneeling half hour, repeated bending for 7 hours, both legs, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, ability to distinguish basic colors and shades of colors, hearing without aid, mental and emotional stability, and working with detergents and antiseptic solutions.
