Job Summary: Provides help with all aspects of patient care, including diagnosis, treatments and consultations as part of a physician/nurse team. Works in outpatient situations and can perform independently or as part of a treatment team. Serves as an educator, case manager, consultant to staff and multidisciplinary health care team. Training and experience in Functional Medicine required.
Key Position Details: This position will be part of a Functional Medicine micro-team consisting of an integrative/functional medicine clinician and an acupuncturist.
Job Description:
Principle Responsibilities
- Provides clinical care for patients using an integrative/functional medicine philosophy
- Provides clinical expertise in integrative/functional medicine.
- Provides advanced clinical consultation and education to patients.
- Performs integrative consultations on patients with acute or chronic health problems including physician, social, emotional, mental and spiritual aspects of health and wellbeing.
- Develops integrative approaches to health promotion, disease prevention and chronic disease management.
- Utilizes both conventional and integrative medicine to emphasize optimal health and wellbeing.
- Participates in health care team
- Promotes communication within the team and other interdisciplinary health care staff.
- Acts as resource to members of the team.
- Works with primary care team for program development.
- Acts as liaison with outside agencies as needed.
- Other duties as assigned.
Job Requirements
- Master's degree Completion of accredited Nurse Practitioner program required.
- Minimum 2 years experience as an Integrative Medicine Provider or in the process of completing a certification in Integrative/Functional Medicine.
- 2 years minimum experience post Nurse Practitioner Training required.
- Experience in providing Integrative Medicine consultations strongly preferred.
