We are seeking a dedicated and compassionate Psychiatric Nurse Practitioner to join our healthcare team, providing specialized mental health services to residents in senior living facilities. The Psychiatric Nurse Practitioner will be responsible for rounding on patients in nursing homes, assisted living facilities, and other senior living communities, offering psychiatric evaluations, medication management, and therapeutic support. The incoming provider will commit to the highest quality of care using an organized, multidisciplinary team approach to provide continuity of care throughout the post-acute continuum. Our patient population served includes adults and geriatrics.
Duties:
- Provide on-site psychiatric assessments and medication management to patients in skilled nursing, assisted living, and other long-term care facilities.
- Coordination of care with attendings, consults, referral sources, and staff.
- Perform physical exams, obtain complete medical history, and record findings.
- Order appropriate laboratory and diagnostic procedures.
- Interprets and integrates data to determine preliminary diagnosis and therapeutic plan.
- Order/administer medication and injections as deemed medically necessary.
- Instruct patients/families regarding medications, post-admission care and treatments.
- Educate patients regarding health and illness prevention.
- Collaborate with physicians in managing acute needs of patients.
Qualifications:
- Graduation from an accredited Nurse Practitioner program.
- ANCC certification as a Psychiatric Mental Health Nurse Practitioner.
- DEA license.
- BLS & ACLS certified.
- Current State license.
