This part time position will involve traveling to see patients onsite and in the community of our Everett and Redmond sites.
The Physical Therapist (PT) is responsible for PT evaluations and the development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and is responsible for communicating and coordinating care with the team, as well as multiple outside providers, caregivers, and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assisted Living Facilities, and nursing homes. The PT assumes responsibility for participants treated by the PTA and PT aide.

Required Qualifications:
- Bachelor's Degree in Physical Therapy.
- Within 30 days of hire: National Provider BLS - American Heart Association.
- Upon hire: Washington Physical Therapist License or California Physical Therapist License (if practicing within this state).
Preferred Qualifications:
- Master's Degree in Physical Therapy or D.P.T. in Physical Therapy.
- 1 year of experience working with a geriatric population.
