Physical Therapist (PT) - Part-time up to Full-time
We are seeking a dedicated and skilled Physical Therapist to join our team. Essential Duties and Responsibilities:
- Review available resident information and evaluate the impact of this information on the assessment and treatment process.
- Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures including assessment of home/vocational and equipment needs.
- Develop and implement individualized treatment plans with achievable functional goals.
- Maintain accurate and timely department records, adhering to evaluation, treatment, and documentation guidelines.
- Instruct and supervise Assistants in treatment procedures, ensuring close oversight of their work with residents.
- Continuously revise treatment plans as necessary throughout the treatment process.
- Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format.
- Orient, inform, and train residents and families about ongoing exercise programs and therapeutic procedures for continued care at home.
- Assist in the referral process when a resident requires additional services following discharge and maintain knowledge of community resources.
- Participate in rehabilitation team conferences, department/facility meetings, and continued education to maintain clinical competency.
- Act in compliance with regulatory and professional standards, ensuring adherence to facility policies and procedures.
Minimum Position Qualifications:
- Bachelor/Masters of Science degree in Physical Therapy.
- Current physical therapy license in the state of practice.
- Experience, training, or supervision in settings serving similar resident populations.
- Excellent communication, interpersonal skills, and a strong ability to work independently or as part of a team.
- Demonstrated competency in physical therapy principles, methods, materials, and equipment.
- Understanding of OBRA regulations, HIPAA, and third party reimbursement guidelines.
Preferred Qualifications:
- Certification in specialty areas of physical therapy.
- Knowledge of accreditation standards and compliance requirements.
- Prior experience in a rehabilitation setting or similar facility.
- Strong assessment skills with the ability to develop and implement effective care plans.
- Proactive in continuing education and professional development.
Skills:
- Advanced skills in patient evaluation and treatment.
- Expertise in developing and supervising exercise programs.
- Strong organizational skills with the ability to manage multiple tasks simultaneously.
- Ability to engage and motivate patients throughout their treatment process.
