Job Overview
The Registered Nurse Case Manager plays a critical role in coordinating patient care and ensuring that individuals receive the necessary medical services throughout their treatment journey. This position requires a compassionate and organized professional who can effectively manage patient cases, communicate with healthcare providers, and advocate for patients' needs. The ideal candidate will possess a strong background in nursing, case management, and familiarity with various healthcare settings.
Duties
- Document patient status and communicate appropriately with Interdisciplinary team members and/or physicians;
- Assess patient and family needs to provide comprehensive plans of care;
- Coordinate end-of-life healthcare and social services for patients and their families;
- Assist patients and families in appropriate continuity of care plans, in-home and in the facility;
- Furnish services requiring substantial and specialized nursing skill. Initiate appropriate palliative nursing procedures;
- Participate in IDG team meetings/care conferences and communicate with other hospice team members involved in care;
- Manage and supervise LPNs and CNAs performance in implementing nursing services;
- Regularly re-evaluate the client’s nursing needs and revise the plan of care as necessary;
- Perform on-call responsibilities as directed by Agency Management;
- Provide nursing care to patients as prescribed by the physician and/or approved care plan;
- Instruct, supervise, and evaluate home health aide care provided at least every two weeks;
- Aid patients in daily living tasks to maximize comfort and enable increased self-sufficiency;
- Implement teaching specific to patient/family needs and evaluate and modify as needed.
Skills
- Strong understanding of HIPAA regulations to maintain patient privacy.
- Proficiency in medical terminology relevant to various healthcare disciplines.
- Experience in case management practices within managed care environments.
- Knowledge of infectious disease care protocols and critical care procedures.
- Ability to perform diagnostic evaluations effectively in diverse clinical settings such as ICU or outpatient facilities.
- Excellent communication skills for effective interaction with patients, families, and multidisciplinary teams.
- Strong organizational skills to manage multiple cases simultaneously while ensuring high-quality patient care.

Pay: $70,566.00 - $83,928.00 per year
