Hospice Nurse Case Manager - Full time
We are actively seeking a Hospice Nurse Case Manager to join our dedicated team. This role is crucial in providing top-level care and support to terminally ill patients. By managing all facets of patient care, you will oversee the coordination and administration of comfort care, ensuring physical, emotional, and spiritual support is given. Your role will effectively alleviate the stresses on both patients and their families during critical times, highlighting our commitment to respect, dignity, and quality of remaining life.

Essential Duties and Responsibilities:
- Oversee the comprehensive care of terminally ill patients, ensuring a coordinated and compassionate approach.
- Collaborate closely with physicians and other medical professionals to fine-tune care plans and treatments.
- Administer medications and manage orders as prescribed by attending healthcare professionals.
- Arrange and prioritize patient visits based on medical and emotional needs, ensuring timely and effective care.
- Supervise and provide guidance to LPNs, LVNs, and Hospice Aides, maintaining high standards of care.
- Deliver emotional and practical support to both patients and their families, educating them on symptoms management and safe patient care practices.
Minimum Position Qualifications:
- Associate's degree in Nursing or related field.
- Valid Registered Nurse license.
- A minimum of one year of nursing experience.
- Prior experience in hospice or palliative care settings.
- Ability and willingness to work on-call as needed.
- Must possess reliable transportation and a valid driver's license for home visits.
Preferred Qualifications:
- Bachelor’s degree in Nursing preferred.
- Exceptional interpersonal communication skills.
- Strong organizational and leadership abilities.
- Demonstrated capacity for empathy and compassion.
- Experience in pain and symptom management in a palliative care context.
- Familiarity with electronic medical records and mobile communication tools.
Skills:
- Proficient in managing care plans and medication protocols.
- Capable of high emotional intelligence to support patients and families in distress.
- Effective supervision of ancillary staff and coordination with medical teams.
- Strong decision-making skills under pressure
