The Department of Medicine – Pulmonary has an exciting opportunity for a Pharmacist. Incumbent will remain committed to implementing an outpatient clinical pharmacy services program.
Responsibilities:
- Collect data on the impact of the clinical pharmacy services provided.
- Learn CFF Port Patient registry concepts, methods, data entry, and reporting on an ongoing basis.
- Track and report on patient assessments, subsequent assessments and follow-up, provision of interventions, and their outcomes.
- Document in the annual progress reports, the CFF Port Registry, and in the patient(s) Electronic Medical Record (EMR).
- Update and notify of any changes to the CF Patient Registry protocol to adhere to CFF guidelines and protocols.
- Learn and advise about the effectiveness and safety of drug combinations in patients.
- Assist with Prior Authorizations of medications and needed follow-up.
- Coordinate pharmaceutical care with third party vendors and/or specialty pharmacies.
- Assist in establishing Standard Operating Procedures (SOPs) and other meaningful guidelines for our center from a pharmaceutical perspective.
Minimum Requirements:
- Minimum 1 year of relevant experience.
- Skill in collecting, organizing, and analyzing data.
- Ability to recognize, analyze, and solve a variety of problems.
- Ability to exercise sound judgment in making critical decisions.
