Location: Multiple Locations - traveling to clients (MILEAGE REIMBURSEMENT INCLUDED) & TELEHEALTH

Schedule: Full Time: Monday - Friday; with OPTIONAL on-call

Compensation: $120-145K

Additional Compensation: $8-12K (approximate, additional pay comes in for after-hours calls, mileage, etc.)

Position Overview: Nurse Practitioners are key members of Lifespark Health's professional medical practice. We deliver primary care and consultative services to frail and complex patient populations. This currently includes home-based primary care in the community and on senior housing campuses, attending physician and APRN services in transitional care units, and in our innovative Hospital at Home program. We utilize a combination of in-person and telehealth visits inside a multidisciplinary team-based model. Patients and their caregivers receive expert, accessible, and compassionate care.

Qualifications:

Current Minnesota APRN or PA license with current board certification
Current DEA license
Certification in gerontology/geriatrics preferred, or strong experience in primary care, community based, and/or palliative medicine
Meet threshold criteria/qualifications for Credentialing and Privileges.
Communicates effectively in verbal, written, and electronic methods.
Able to work in non-traditional settings (client homes, nursing homes, and from home/Telehealth)
