Seeking a Full-Time Nurse Practitioner for CHD's Crisis Service lines. Ideally, double board certified as a Psychiatric Mental Health Nurse Practitioner and Family Nurse Practitioner or Adult Gerontology Nurse Practitioner, to work in a team environment providing care to adults and adolescents in CHD’s Crisis Stabilization and Outpatient Clinic in Chicopee, Massachusetts. These programs treat individuals and families throughout the care continuum. Services include but are not limited to individual counseling, psychiatry, recovery coaching, case management, treatment for substance use disorders, and peer supports.
Essential duties for these roles may include:
- Psychiatric evaluation(s), assessment, diagnosis and treatment of adults/adolescents experiencing symptoms of behavioral health disorders in the short-term crisis stabilization unit.
- Outpatient psychiatric care.
- Collaboration with other physicians, advance practice nurses, psychologists, social workers, nurses, and other professionals.
- Provide clinical consultation, in person or remotely, to other members of the clinical team.
- Collaborate with guardians, relatives, and significant others of people served when clinically indicated.
- Counsel people served and their families when clinically indicated.
- Document evaluations, assessments, diagnosis, and treatments in the electronic health records.
- Participate in individualized treatment plan designs.
- Order, review, and interpret laboratory and other diagnostic tests.
Qualifications:
- State certification as a Psychiatric Mental Health Nurse Practitioner or Clinical Nurse Specialist required.
- State certification as a Family Nurse Practitioner or Adult Gerontology Nurse Practitioner preferred.
- An unrestricted license to practice as an Advanced Practice Registered Nurse (APRN).
- An unrestricted DEA registration.
- A state issued Mass Controlled Substance Registration.
- Registration with the Mass Prescription Monitoring Program (PMP).
- No suspension/exclusion from participation in federal health care programs.
- X-Waiver preferred.
