Function
The Community Health Worker (CHW) will work with specific target populations to identify and address Social Determinants of Health (SDoH). CHWs will collaborate closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
Under the direction of the Nursing Leadership, Care Coordination Nurse Manager, and the Nurse Care Coordinators, the CHW will provide patient support and specialized assistance at all Fenway Health clinical locations.
Principal Duties and Responsibilities:
Care Coordination
- Support the Nurse Care Coordinators with care coordination requests.
- Provide coverage of care coordination duties as needed.
- Assist patients with completion of paperwork.
- Provide patients with updated and accurate information regarding care coordination services.
- Build good working relationships with clients and colleagues.
- Assist patients with applications for enrollment in programs as needed.
- Document in EHR for each patient interaction.
- Assist with care coordination administrative tasks such as completing and faxing client paperwork.
- Make outgoing phone calls to community resources as needed.
- Distribute program materials to clients via mail as needed.
- Convey the purposes and services of a program to the patient population.
- Help patients develop health management plans and goals.
- Follow up with health management/care plans with both patients and providers.
- Coach patients in effective management of their chronic health conditions.
- Assist patients in understanding care plans and instructions.
- Document activities, service plans, and results effectively while adhering to policies and procedures.
- Establish positive, supportive relationships with participants and provide feedback.
- Assist patients in accessing health-related services.
- Facilitate communication and coordinate services between providers.
- Motivate patients to be active participants in their health.
- Build and maintain positive working relationships with clients and staff.
- Continuously expand knowledge of community resources and services.
Attends to healthcare issues of specific populations
- Provide age-specific, culturally responsive care per Fenway Community Health policy.
- Work with the care team to complete processes for patient benefits.
- Conduct outreach and coordinate with medical case managers.
- Provide a high degree of customer service.
- Outreach to patients to obtain necessary information.
- Handle sensitive medical and behavioral health information with care.
Provides health education
- Provide community health education.
- Provide patient education related to their medical and psychosocial needs.
- Maintain comprehensive knowledge of vaccines, medications, and therapeutic interventions.
- Access Nursing Department resources to ensure up-to-date information for patients.
- Maintain knowledge of Fenway Health policies and community resources.
- Assist in training other employees in the medical department.
Effectively communicates with patients and co-workers
- Work with diverse backgrounds to reduce cultural and socio-economic barriers.
- Demonstrate competency in EMR, Microsoft Outlook, etc.
- Participate in staff meetings and Nursing meetings.
- Be available to meet with team members during assigned hours.
- Understand and abide by HIPAA regulations.
- Demonstrate a commitment to strong customer service.
- Work effectively as a team member.

Requirements
- High School Diploma.
- Knowledge of MS Office suite, particularly Excel and Outlook.
- Strong organizational and attention to detail skills.
- Ability to handle multiple tasks and prioritize duties.
- Ability to work harmoniously with diverse groups of individuals.
- Willingness to contribute towards Fenway's efforts in becoming an anti-racist organization.
Preferred Qualifications:
- AS or BS in psych/public health or related field.
- Office management experience.
- Experience with LGBTQ patient populations.
- Work with addressing SDoH.
- Relevant administrative experience in a Behavioral Health/Public Health and/or medical setting.
- Experience working with a diverse staff.
This is a union position in a Fenway Health bargaining unit represented by 1199 SEIU United Healthcare Workers East.
