Clinical Pharmacist - Aurora - Breakthrough Care Clinic

Responsibilities:
The scope of the Breakthrough Care Clinic Pharmacist shall include only those clinical services the Collaborating Physician generally provides to his or her patients in the normal course of clinical practice.
- Access and update patient medical records and important health information that may influence clinical decisions.
- Evaluate therapeutic regimens based on efficacy, safety, tolerability, drug interactions, cost, patient preference, and professionally recognized clinical guidelines.
- Interview patients and perform targeted physical assessments as deemed necessary to assess response to therapy.
- Provide patient education regarding disease states, self-monitoring, and medication therapy.
- Order laboratory tests to aid in monitoring drug therapy.
- Evaluate patient response to pharmacological interventions and provide verbal and/or written recommendations to the patient and/or caregiver for modification of drug therapy as clinically indicated.
- Schedule and complete MTM clinic follow-up as deemed necessary by the PharmD.
- Initiate, adjust, or discontinue insulin therapy.
- Adjust warfarin therapy.
- Initiate, adjust, or discontinue lifestyle recommendations.
- Discuss interventions with the patient’s caretakers in accordance to HIPAA.
- Document patient encounters in the electronic medical record in accordance with Duly Health and Care Clinic Policies and Procedures.
- Perform administrative functions to maintain Duly Health and Care clinical pharmacy services.
- Conduct quality assurance assessments for clinical pharmacy services.
- Respond to drug information requests from healthcare providers.
- Provide Duly Health and Care staff education and development as requested.
- Complete MTM Outcomes.
Orders may be given in writing or verbally, including via telephone. Any verbal order must be given only to those individuals who are authorized to accept such orders, in accordance with Duly Health and Care policy. Any telephone order must be signed as soon as reasonably practicable. Collaboration must take into account the PharmD’s training and education.
Qualifications:
EDUCATION and/or CERTIFICATION/LICENSURE:
- Doctor of Pharmacy (Pharm.D.) degree from an accredited College of Pharmacy required.
- Current Illinois State Pharmacy License required.
EXPERIENCE:
- 3+ years of experience
- MTM experience preferred
