Home Palliative Care Nurse Navigator is primarily responsible for providing comprehensive support and coordination of care for home patients needing palliative care services. This role involves conducting home visits, assisting with virtual visits, managing durable medical equipment (DME) orders, triaging clinic calls, and responding to after-hours calls. Additionally, the Nurse Navigator assists with clinic duties (Tuesdays and to cover PTO’s) including rooming patients, taking vitals, and ensuring clinic rooms are stocked with necessary materials.
Qualifications
- Education: Bachelor's degree, preferred. RN required.
- Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.
- Licensure/Certification: Current nursing license in appropriate state, required.
- Additional skills required: Ability to successfully complete generic and department-specific skills validation and competency testing.
