_*Job Responsibilities:*_
* Conduct a thorough and precise assessment of patient condition, functional abilities for a complete and thorough initial nursing evaluation of the patient.
* Make appropriate referrals for additional services such as Nursing, OT, etc.
* Develop, coordinate and revise the treatment plan based on physician recommendations.
* Provide care in accordance with physician orders, the patient’s condition, accepted standard of practice and agency policies.
* Follow patient for weekly visits per patient's needs and physician orders
* Must complete paperwork, prepare accurate timely reports and required documentation in all applicable systems per agency standards and protocols.
* Adhere to all practice standards and best practices as they apply to patient care.
_*Job Requirements:*_
* Valid PT License in State of MO
* MO Drivers License with the ability to drive within the given territory
* Valid auto insurance.
* Excellent charting skills (Axxess - preferred)
* Home Health experience preferred
