Summary: The intent of DM 3700 is to cut Medicaid costs while not compromising care by outreaching, engaging, and enrolling high medical and behavioral health utilizers in CPRP and HCH programs to help them better manage their mental and physical health conditions. Individuals that are Medicaid eligible with high Medicaid costs, including frequent Emergency Department utilization, are assigned to Swope Health based on the individual’s last address on file. Individuals are placed on the Swope Health DM Cohort list for outreach based upon catchment area, or the individuals may have been identified as currently receiving some medical or behavioral health services at Swope Health.
Primary Functions:
- DM 3700 Case Manager obtains SHS DM Cohort from Care Manager and reviews the Cohort upon receipt, prioritizes individuals, and begins outreach.
- Outreach efforts include any or all of the following:
- Welcome post card sent to the individual address indicating services provided and contact information for the DM Case Manager
- Phone Calls to phone numbers available for the individual
- In person home visits to meet the individual face-to-face
- Visits during an inpatient hospitalization to engage the client in services
- Networking with other community providers in effort to engage the individual at their locations
- Connecting with a primary care physician to engage the individual
- Contact with a pharmacy to engage the individual
1. DM Case Manager makes contact with the individual and provides targeted case management in attempt to engage/enroll in necessary services to address physical health and behavioral health needs to include substance use disorders. These services include: Community Support; Healthcare Home; Psychiatry; Psychosocial Rehabilitation Program; Dental; Optometry; Wellness Coaching.
2. The DM Case Manager may also assist the individual with applying for lifeline benefits, housing support, as well as connecting to resources for other basic needs.
3. DM Case Manager updates status information and provides reports on each cohort monthly in statewide database Care Manager.
4. The DM Case Manager documents all outreach efforts and progress notes in the electronic medical record for the individuals they outreach and engage.
5. DM Case Manager facilitates HCH enrollment and MSS completion.
6. DM Case Manager assists with scheduling/completing BH Intake Into Care (if applicable), Psychiatric Evaluation (if applicable), and Primary Care Establish Care (if applicable).
7. DM Case Manager facilitates warm handoff to permanently assigned CSS once BH services are established.
8. DM Case Manager will act as temporary case manager for anyone who would otherwise be placed on a waiting list for CPRP until a CSS is assigned.
Qualifications:
Bachelor’s Degree in Social Work, Human Services, Psychology, Sociology, or related field.
Minimum of two years case management experience (preferably with high acuity populations).
Knowledge of community resources for the unhoused, individuals experiencing food insecurity, individuals needing rent and utility assistance.
Good interpersonal and communication skills.
Experience working with at-risk populations who utilize emergency rooms for primary medical and behavioral health needs.
