Job Summary
Join our compassionate team as a Registered Nurse (RN) where you'll provide direct patient care in alignment with physician orders and company policies. You'll play a vital role in enhancing patient comfort and quality of life, working closely with our dedicated team to achieve positive outcomes for those we serve.
Essential Functions
- Comprehensive Assessments: Conduct initial and ongoing assessments of patients and families to identify hospice needs, including detailed physical evaluations and medical histories.
- Direct Skilled Nursing Care: Deliver high-quality in-home nursing care, utilizing all elements of the nursing process to support patients and their families.
- Collaborative Care Planning: Regularly assess and evaluate patient status, writing and revising care plans as necessary to meet evolving needs.
- Interdisciplinary Coordination: Plan, coordinate, and deliver exceptional interdisciplinary home health care services, ensuring compliance with all standards and regulations.
- Documentation: Ensure timely completion of documentation, including obtaining necessary consents and submitting them promptly to intake.
- Care Plan Initiation and Revision: Initiate and adapt care plans based on ongoing assessments, incorporating patient and family feedback.
- Nursing Diagnoses: Utilize health assessment data to develop nursing diagnoses and establish targeted care goals.
- Patient and Family Counseling: Provide guidance to patients and families to help meet their nursing and related needs.
- Health Education: Educate patients on self-care techniques and provide instructions on medications, diets, and health management.
- Daily Living Support: Assist patients with daily living activities, promoting independence and comfort.
- Case Management: Act as Case Manager when assigned, coordinating care for an assigned caseload.
- Clinical Notes: Maintain accurate and timely clinical notes reflecting patients' conditions and care provided.
- Communication: Collaborate with physicians and community health personnel to ensure seamless care coordination.
- Interdisciplinary Collaboration: Work closely with the interdisciplinary team to enhance patient care.
This overview highlights the key responsibilities of the position, and all colleagues are expected to perform duties as assigned by their supervisor.
Compensation: $78,000 - $90,000, depending on experience.
Education and Experience
- Qualifications: Graduate of an accredited nursing program (BSN preferred).
- Experience: Minimum of one (1) year as a professional nurse; hospice experience is preferred, along with knowledge in aged, chronically ill, restorative, or psychiatric nursing.
Certifications and Licenses
- RN License: Valid RN license in the state of practice required.
- Driver's License: Must be a licensed driver with a reliable, insured automobile.
- BLS Certification: Active BLS for healthcare professionals from the American Heart Association or Red Cross.
