Work Schedule:
80% FTE, 8 hour days, 4 days a week, core hours of 8:00am to 5:00pm. Hours may vary based on the needs of the department.
As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients.
We are seeking a Nurse Practitioner to:
- Work in a new and innovative Home-Based Primary Care program working with older adults that are homebound.
- Collaborate with a focused interdisciplinary team to care for patients.
- Conduct comprehensive or episodic health history and physical assessment of patients/families with acute, chronic and/or potential health problems in the patient’s home.
- Provide appropriate direction to program and clinic staff regarding patient care issues.
Education:
Minimum –
If Nurse Practitioner:
Master’s or Doctorate’s degree from accredited Nursing Program.
If Physician Assistant:
Bachelor’s or Master’s degree from accredited PA Program.
Licenses and Certifications:
Minimum -
If Nurse Practitioner:
• Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND valid WI Drivers license and reliable vehicle.
• Active board certification as a NP in area of clinical practice. These may include the following: Adult; Adult/Gerontology Primary Care; Family; Gerontology.
If Physician Assistant:
• Active PA license, including prescriptive authority, in state(s) of practice AND
• PA board certification.
All Nurse Practitioner and Physician Assistant applicants must have:
Active DEA license or ability to apply for such license prior to or upon hire.
*Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.
