Job Overview
As the Population Health Manager, you will lead and oversee population health projects aimed at driving data-driven, integrated care improvements. Your role includes managing the Population Health team and collaborating with the Senior Director of Quality and Population Health to identify and tackle major health and quality improvement challenges. You'll work closely with internal departments to drive innovation across Cascadia's services.
Essential Duties
- Lead and manage population health projects, ensuring alignment with the strategic plan.
- Supervise and support Population Health Research staff.
- Collaborate on new projects, services, reports, and initiatives, including budgeting.
- Provide leadership in data reporting and interpretation to improve care.
Qualifications
- Bachelors Degree with 5 years of related experience or Masters Degree with 3 years of related experience, preferably in a leadership role.
- Understanding of recovery models, harm reduction, and trauma-informed care.
- Experience with best practices, population health, and integrated care.
