Work Schedule:
This is a full-time, FTE 1.0 Day/Evening position, with hours from 7:00 am – 3:30 pm / 8:00 am – 4:30 pm, rotating evening shifts from 1:30 pm – 10:00 pm, and every 4th weekend. Hours may vary based on department needs. You will work at University Hospital in Madison, WI.

We are seeking a Clinical Pharmacist - Transplant to:
- Provide medication therapy management services as part of the interdisciplinary transplant team to patients on inpatient and ambulatory services to assure that drug therapy is safe, effective, efficient, and cost-effective.
- Perform both clinical and distributive tasks necessary to ensure that patients’ medication therapy is safe, effective, timely, and efficient.
- Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences.
- Provide patient care to a diverse population including kidney, liver, pancreas, heart, and lung transplant medical and surgical patients.

Minimum Requirements:
Education:
- PharmD degree or equivalent experience Required.
Work Experience:
- Prior pharmacy-related work experience Preferred.
Licenses and Certifications:
- Licensed or eligible for licensure to practice pharmacy in the State of Wisconsin Upon Hire Required.
- Basic Life Support (BLS) certification Upon Hire Required.
