The Care Manager-Outpatient Complex Care position is 0.6 FTE scheduled between the hours of 7:00am - 5:00pm. This nursing position will support the care coordination needs for patients enrolled in Gillette's Complex Care Program. This is a remote opportunity; however, the selected candidate must reside in Minnesota or Wisconsin and have the ability to work onsite in St. Paul, MN as needed.
This position proactively partners with patients and their caregivers to address obstacles in achieving optimal health. The care manager will educate and empower patients and caregivers to facilitate the achievement of optimal outcomes, and facilitate communication between patient, healthcare team, and community resources.
Coordinates services and resources needed to reach the identified goals. Continuously evaluates the effectiveness of the care management plan with the patient, caregivers, the healthcare team, and community resources, modifying as needed. Communicates with the patient, family, and healthcare team regarding progress on the plan of care.
Compensation & Benefits Information:
The annual salary range for this opportunity is $54,637.44 to $81,956.16, with a median salary of $68,303.04. Pay is dependent on several factors including relevant work experience and internal equity. Salary is just one component of the compensation package for employees. Core Responsibilities and Duties
- Patient Centered Care
- Collects subjective and objective information pertaining to health status from the patient, caregivers, health records, Gillette care team, and community and any relevant sources to coordinate care.
- Utilizes an interprofessional approach to identify goals that are patient-centered, specific, and measurable.
- Provides education and resources appropriate to the individual patient and family to help the patient meet goals.
- Effectively prioritizes with the patient and caregivers strategies to meet goals.
- Communicates patient information to enhance active and informed decision-making by patients, caregivers, and the healthcare team.
- Care Coordination and Collaboration
- Identifies and establishes effective communication methods with patient, caregivers, and health care team across specialties and service lines at Gillette.
- Communicates and collaborates between specialty providers and primary care providers to develop and share a mutual plan of care for the patient.
- Initiates proactive communication with patient, caregivers, and the healthcare team to meet patient needs and care plan goals.
- Communicates and monitors patient care transitions (inpatient, outpatient, ED, primary care).
- Facilitates transition services from pediatric to adult within Gillette or within the community.
- Advocacy
- Proactively negotiates and secures services to meet patient needs and plan of care.
- Coaches patients and caregivers in self-advocacy skills.
- Manages and coordinates referrals with Gillette and community services.
- Relationships and Team Building
- Establishes and maintains good working relationships with all members of the healthcare team.
- Supports other team members in achieving patient-centered goals.
- Assists supervisor in maintaining a cohesive Care Management team by contributing to a collaborative, respectful, and diverse environment.
- Participates in and contributes to appropriate departmental and/or organizational meetings.
- Procedures and Policies
- Identifies, interprets, and implements organizational policies and procedures according to regulations, professional, and community standards.
- Maintains current and accurate documentation in the EHR in accordance with Care Management policy and procedure.
- Protects the confidentiality of personal health information.
- Maintains knowledge of and effectively uses automated applications and systems.
- Identifies deficits in technological literacy and seeks appropriate training under the guidance of a supervisor.
- Maintains maximum individual productivity through proficient use of automated systems.
Qualifications (including KSAOs)
Required:
- Current RN license through the MN Board of Nursing
- A minimum of 5 years recent clinical experience
Preferred:
- Master’s degree in nursing or related field
- Certification in specialty area i.e., Nursing Case Management or Accredited Case Manager
- Care Management and leadership experience
- Current BLS certification (Basic Life Support through the American Heart Association) or obtain within 90 days of hire.
