The ambulatory pharmacy navigator program provides for improved service levels for HMH Discharge Patients by seeking to supply medications prescribed by the patient's physician for discharge in a timely manner so that the patient can be discharged efficiently from an acute care HMH facility and transition directly to home. The Navigator I assists with obtaining all information necessary for prescription processing, helps prioritize workflow, helps troubleshoot barriers (financial, insurance, or clinical), and facilitates delivery of completed prescriptions back to the patient prior to discharge.
The Navigator ensures OBRA compliance by ensuring an offer to counsel is made and that patient medication educational needs are met. The Navigator I functions will include all job functions of an Ambulatory Pharmacy Technician plus additional duties described below.
Employee will do initial training in Eatontown. Once training is completed, Perth Amboy will be the permanent work location.
Responsibilities
A day in the life of a Pharmacy Navigator I at Hackensack Meridian Health includes:
- The Navigator duties include all job functions of an Ambulatory Pharmacy Technician plus additional duties.
- Establish an initial greeting to patients admitted and explain services available via the HMH Pharmacy.
- Perform prescription order entry duties as necessary.
- Obtain patient authorization to process discharge prescriptions through the ambulatory pharmacy.
- Engage in a collaborative process with floor nurses, discharge planners, case managers, social workers, physicians, APN's, PA's, Medical Residents, unit clerks, and other teams/initiatives as required to ensure patients scheduled for discharge to home can receive the critical medications needed in a timely manner.
- Work to determine patients being scheduled for discharge and ensure HMH Pharmacies are selected by patient choice to provide services.
- Determine information sources necessary to process prescriptions (e.g., insurance coverage).
- Identify and troubleshoot discharge patient medication access issues related to insurance, financial or insurance coverage, and help to resolve so patients can receive needed medications.
- Adhere to all established HMH Pharmacy and Federal/State privacy and compliance policies to protect patient confidentiality.
- Ensure prescription fulfillment by in-house pharmacy based upon priority needs to allow for timely discharge.
- Coordinate and facilitate patient pickup of prescriptions and/or delivery of completed prescription orders to patient care areas.
- Ensure patients receive an offer for pharmacist counseling and distribute pharmacist contact information in the event patient has prescription-related questions at home.
- Facilitate distribution of medication-related educational materials and coordinate counseling with staff pharmacist where needed.
- Assist Ambulatory Pharmacy staff as needed.
- Perform duties of Ambulatory Pharmacy Technician if required.
- Ensure all guests/patients receive safe care. Maintain a safety-first work ethic. Report any medication errors or medication variances as required by HMH Pharmacy Policy.
- Other duties and/or projects as assigned.
- Adhere to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills, and Abilities Required:
- High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills, and Abilities Preferred:
- Retail Pharmacy experience.
- Strong knowledge and experience with PBM Claims and coverage.
Licenses and Certifications Required:
- Pharmacy Technician License.
- PTCB or ExCPT Certification Required within 180 days of start of employment with the following exceptions:
- Hired prior to January 2024.
- Actively enrolled in ACPE accredited school of pharmacy / another allied health program or hired before 1-1-24.
