What You'll Be Doing
The licensed practical nurse is expected to perform tasks and responsibilities within the framework of nursing services and the scope of practice in a skilled nursing facility. The LPN is part of the interprofessional team to help lead the resident and family teaching program through health teaching, health counseling, provision of supportive and restorative care, under the direction of a registered nurse or licensed clinician/provider.
- Provide comprehensive care to patients by applying the nursing process in an organized and systematic manner according to policies of the Medical Center and The Transitional Care Unit in keeping with patient safety principles.
- Perform all clinical skills and services as per policy and accepted procedure within the LPN scope of practice.
- Complete clinical competency, required certifications, and peer evaluation process.
- Collaborate with the resident, significant others, ancillary departments, and functions as part of the multidisciplinary health care team whose goal is to provide care that is continuous and well-coordinated with specific discharge goals.
- Perform effectively and efficiently in emergency situations.
- Evaluate and document resident progress/response toward expected outcome(s) and report pertinent observations and recommendations to the interprofessional team.
- Communicate proactively to the interprofessional team the plan of care changes in patient condition, the patient progress, the discharge plan and other relevant information.
- Document resident care and required safety checks accurately and legibly.
- Perform ongoing evaluation of residents’ physical and mental health and notify the interprofessional team and/or clinician/physician, including abnormal lab and diagnostic results.
- Explain medications, procedures, and treatment protocols to residents.
- Administer medications via intravenous, oral, topical, subcutaneous, intramuscular, gastrostomy and other routes of administration within scope of practice.
- Administer enteral feedings via nasal gastric tube or gastrostomy tube.
- Perform bedside care such as wound dressing, changing bandages, inserting catheters, and giving injections.
- Obtain resident's vital signs such as respiration, temperature, blood pressure, and heart rate as needed and document.
- Supervise certified nursing assistants and other auxiliary staff.
- Take verbal orders from the provider/physician and document appropriately.
- Notify physician of any pharmacist consultant’s recommendation for revision of medications, medication alerts, or changes in medication administration times as written in the Electronic Pharmacist Information Consultant (EPIC) form then implement the approved changes.
- Initiate and/or discontinue peripheral IV heplock.
- Call pharmacy for ordering prescriptions when discharging residents home.
- Provide hand-off report to receiving unit when transferring patients.
- Maintain a safe and comfortable environment for residents, visitors, and staff.
- Comply with policies and procedures concerning infection control and safety for all individuals on the unit.
- Maintain and develop competence through required in-service and continuing educational programs.
- Maintain current CPR and IV certifications.
- Discharge patients to home or other healthcare settings as supervised by a registered nurse.
- Be an active and contributing member of the Quality Assurance Performance Improvement Committee.
What We're Looking For
Active NJ LPN license and BLS certified. Preferably a minimum of 2 years’ experience in post-acute care settings (i.e. skilled nursing facility, rehabilitation, long-term care).
