PRN Hospice Nurse Practitioner: Make an Impact with Legacy Hospice

As a Hospice Nurse Practitioner, you'll play a vital role in planning, organizing, and directing hospice care in patients' homes and communities. Key Responsibilities:
- Provide medical care, palliation, and manage terminal illness under the supervision of a collaborative physician.
- Complete examinations, order and interpret test results, diagnose health conditions, and document the findings in the patient’s medical record.
- Manage health conditions by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or referrals, and coordinating hospice care services.
- Make recommendations to and consult with the hospice Medical Director.
- Communicate and collaborate with the interdisciplinary group, the patient’s attending physician, and other community health practitioners.
Key Qualifications:
- Graduate of an accredited nursing school and accredited Nurse Practitioner Program.
- Current nursing licensure in State and CPR certification.
- Master's degree with a minimum of (1) year of Nurse Practitioner experience and a minimum of (1) year of hospice or palliative care experience.
- Accreditation from an approved certifying body for advanced practice nursing as required by the State.
- Certification in a specialist area is preferred.
- Excellent observation, verbal and written communication skills, problem solving skills, mathematical skills; nursing skills per competency checklist.
- Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
