Overview:
Provide information about those drugs and help patients understand the instructions their doctors or other health practitioners provided. Provides supervision to pharmacy technicians.
Duties and Responsibilities:
- Standard pharmacist duties to include prescription intake, verification and patient counseling.
- Advise physicians and other health practitioners on the selection, dosages, interactions, and side effects of medications.
- Evaluate appropriateness of prescription orders with consideration for patient safety and legal requirements.
- Performs drug utilization reviews (DUR’s) and document interventions.
- Monitors the health and progress of patients in response to drug therapy.
- Advise patients and answer questions about prescription drugs, including questions about possible side effects and interactions among different drugs.
- Provides information and make recommendations about over-the-counter drugs.
- Give advice about diet, exercise, or stress management.
- Advises patients about durable medical equipment and home healthcare supplies.
- Supervise pharmacy technicians to ensure compliance with all standards of care to include data entry and preparation of medications and work plans.
- Comply with Avenue 360’s policies and procedures as well as program specific procedures and protocols.
- Demonstrate ability to set priorities and work cooperatively with others within and outside the department.
- Performs all other duties as assigned by appropriate management.

Participates in trainings required by the funding source and/or as required by licensure if applicable.
