Job Summary:
Constellation Health Services is seeking hospice experienced RNs (Registered Nurses) to work per diem!
This role involves providing initial assessment and admission for patients referred for Hospice Services, performing emergent visits and pronouncements, and possible continuous care overnight. The RN (Registered Nurse) supports the philosophy, goals, and culture of Constellation Health Services.
Responsibilities:
- The RN will conduct a comprehensive patient assessment, considering strengths, goals, measurable outcomes, and care preferences.
- Complete an initial assessment and visit required within 48 hours or per physician's orders.
- Perform emergent visits and pronouncements.
- Provide continuous care visits typically overnight.
- Provide patients with detailed written information, including staff schedules, medication dosing, treatment plans, and agency contact info.
- Ensure the inclusion of an emergency plan in patient records.
- Administer and collect signed admission packets.
- Attend mandatory staff meetings, maintain compliant documentation, and utilize a computer-based system for submissions.
- Instruct and supervise clinical personnel and contribute to home health aide evaluations.
- Act as a liaison with healthcare institutions for seamless patient care transitions.
- Represent the agency in community activities and participate in Interdisciplinary Team (IDT) meetings as necessary.
Qualifications:
- Graduate of an accredited RN program.
- Connecticut State Licensed Registered Nurse with a minimum of one year of clinical experience.
- Previous home care/hospice experience is required.
- Knowledge of hospice regulations and qualifications for hospice admission.
- Excellent observation, good nursing judgment, and communication skills; understands hospice philosophy and issues of death/dying.
- Must possess basic computer skills, excellent documentation, and customer service skills. Previous experience with MatrixCare is a plus!
