The Cadence Health team seeks a Licensed Practical Nurse to support our newly established Chronic Care Management Program. The LPN will provide care, education, and support to our CCM patients to better manage their conditions. This will be our first team member supporting CCM; you will partner with our Operations and Clinical Leadership team to build out the program as we integrate it into our current RPM program.
This role's schedule will be M-F 8am to 5pm Pacific Time.
WHAT YOU’LL NEED:
- Multi-state LPN Compact State Licensure is required.
- 5+ years of clinical experience in chronic care management required.
- Previous experience leading or building a CCM program.
- To ensure that our clinicians have the necessary tools for a successful remote work environment, home office setups must have consistently stable wifi with strong upload and download speeds. A wifi speed test is required before participating in the interview process to verify that these standards are met.
- Previous experience in a high-growth, fast-paced, autonomous environment.
- Skilled in nursing processes.
- Excellent clinical acumen and high competency in patient assessment for patients managing chronic conditions.
- Exceptional written, verbal and interpersonal communication skills.
- EMR experience, preferably in Athena and EPIC.
- Works effectively with minimum supervision and demonstrates ability to problem solve.
- Strong team collaboration skills.
- Ability to support delivery of health care to patients by performing a variety of activities and procedures which are prescribed by and performed under the direction of the Cadence Nurse Practitioner and Cadence clinical policies and procedures.
- Ability to consolidate data from multiple sources into a singular, coherent platform with clarity and efficiency.
- Tech fluency; we are a remote organization and work across multiple platforms and systems, including Notion, Athena, EPIC, Zendesk, G Suite, etc.
