The Family Nurse Practitioner will support and initially treat any medical needs required, traveling when necessary to each New York City location. The Family Nurse Practitioner will conduct an initial medical assessment, refer medical services to NYC Programs as needed, and serve as point of contact for all 29i related needs.
HOURS PER WEEK: Exempt
- Regular hours entail 10:00am - 6:00pm or 9:00am - 5:00pm, Monday - Friday with some weekends and holidays required.
- Must be flexible to meet program needs.
FUNCTIONS:
- Conduct initial medical assessments for each youth in the New York City Programs.
- Diagnose and treat basic medical conditions for each youth.
- Provide On-Call Support.
- Refer New York City Program youth to appropriate medical specialties.
- Assessment of youth as requested by programs and stakeholders.
- Write prescriptions.
- Administer injections and immunizations; suturing; managing wounds and infections.
- Conduct follow-up services for New York City programs.
- Develop and oversee the NYC vaccination program ensuring all regulations are met.
- Administer vaccinations as needed for each New York City Program.
- Create and electronically submit required medical paperwork (insurance, billing) for each New York City Program.
- Create and update information for each New York City Program youth web-databases.
- Ensure proper documentation is completed for medical services.
- Work collaboratively with program staff and treatment teams to provide appropriate services to youth in the New York City Programs.
- Work collaboratively with the Case Planner, Clinician, and Behavioral Support Specialist to provide appropriate services to each youth in the New York City Programs.
- Maintain up-to-date and enforce the stakeholders’ policies surrounding medical needs for youth.
- Ensure Cayuga Centers is completing assessments and reports within each stakeholder’s timelines.
- Discuss medical conditions with the New York City programs’ stakeholders.
