Make a Difference
The diabetes educator provides an educational experience for patients and their significant others to promote effective diabetes self-management. This role is multidimensional with accountability to self, patient, profession, and other members of the health care team. Responsibilities include:
- Assessing patient strengths and educational needs using available data and resources.
- Collaborating with the team to establish and prioritize individualized plans with patients and their significant others.
- Incorporating generally accepted technology and knowledge into patient education experiences based on standards of practice for own discipline, ADA, AADE, and other regulatory agencies.
- Promoting safe practices of diabetes self-management related to medication administration, meal planning, exercise, blood glucose monitoring, and other aspects of care.
- Empowering patients and their significant others with knowledge and skills in self-care for optimal health and psychosocial adaptation to maintain/improve quality of life.
- Consulting with other health care professionals and organizations to utilize appropriate resources (human, financial, and material) which promote consistency and continuity of care of high value.
- Performing other related tasks as needed.
The Diabetes Management clinic is located on the north side of Indianapolis, Indiana; however, some travel will be required to the East side of Indianapolis and Carmel.
Exceptional Skills and Qualifications
- Graduate of an NLN, CCNE, ACEN, or NLN CNEA accredited school of nursing, or three years of related professional nursing experience. Bachelor’s degree in nursing required.
- Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).
- Two years or more of nursing experience required.
- BSN required.
- Certified as a diabetes educator preferred before hiring.
- One to two years of educating patients in the area of diabetes preferred.
- Excellent team skills.
- Compassionate and caring.
- Accountable and self-directed.
