Clinician, Adult Services: Half time in our Adult Outpatient Clinic providing individual, group, and case management services to adults. Half time providing services off site for Acute Community Support Team (ACST) intended to treat, stabilize, and support clients with Severe and Persistent Mental/Emotional Illness whose symptoms tend to be barriers and interfere with access to treatment. We work to stabilize with the hope of eventual return to outpatient care after stabilizing and help them build a better quality of life through access to care.
KEY RESPONSIBILITIES
- Conduct comprehensive clinical assessments and engagement.
- Determine DSM V Diagnosis in consultation with supervisor, Medical Director, Psychiatrist, medical provider, or designee as needed.
- Develop treatment plans and contracts for services.
- Implement treatment plans, modifying them as appropriate during the course of treatment.
- Discharge planning, which may include linkages to community support services.
- Provide case management services.
- Complete clinical documentation required by agency and program policies, HIPAA regulations, and funding sources.
- Maintain productivity targets determined by service.
- Participate and present in Peer Consultation and Quality Assurance Reviews.
- Foster collaboration with other clinical and administrative staff to respond to client and programmatic needs.
- Identify and develop approaches to address clients’ unmet needs or service gaps.
- Acquire and maintain the needed skills in evidenced-based models that the agency/program ascribe to.
- Partner with Supervisor to identify and develop plans for professional growth.
- Contribute to program functioning by assuming responsibility for special tasks or leadership in special projects as appropriate.
- Participate in agency-wide task forces, committees, or problem-solving teams as appropriate.
- Represent the Village in community groups or committees.
Experience and Skills
EDUCATION/QUALIFICATIONS
- Master’s degree in social work, psychology, or other licensed counseling degree.
- State of CT license required (i.e. LCSW, LPC, LMFT, Psychologist).
- Minimum of 2+ years of experience postgraduate in an outpatient setting (depending on program).
- Cultural competency to serve a diverse population.
- Strong diagnostic, assessment, and treatment skills for individual, family, and group therapy.
- Extensive experience working with culturally diverse populations, a wide age range, and a variety of clinical presentations.
- Knowledge of behavioral health, child welfare systems, and community resources.
- Good organizational and communication skills.
- Ability to function autonomously in day-to-day responsibilities; yet seek consultation appropriately when needed.
- Proficiency with word processing and general computer skills.
