The Nurse and Patient Coordination Manager’s responsibilities include the oversight of Nurses and Patient Care Coordinators within the organization. The Nurse and Patient Coordination Manager facilitates/coordinates safe, high quality clinical care, treatment, services and workflows for the organization in collaboration with Senior Management, and other clinical managers and support staff.
- RN with minimum of 2 to 4 years management experience required, BSN preferred.
- Current New Hampshire RN License required.
- Must maintain a current BLS Certification.
Experience: Community health experience preferred. Possess an understanding of compliance requirements and be responsible for reporting on clinical tracking.
Knowledge/Skills: Ability to delegate tasks to other team members. Basic computer skills to utilize software tracking systems. Ability to work independently, decision making ability, ability to delegate effectively, team building experience, strong interpersonal communication skills, coaching experience, strong organizational skills, attention to detail and follow-through as well as a sound clinical knowledge base.
Compensation: The starting rate for this position is $81,203/year which reflects the beginning of the pay scale. At the time of an offer, determination of your rate will reflect your skills and experience as it relates to the position.
Responsibilities:
Works as an integral member of the health care team.
PRIMARY MANAGEMENT FUNCTIONS:
- Oversight of Nursing and Patient Care Coordination staff to include hiring, coaching, mentoring, discipline and evaluation.
- Ensures sufficient staffing resources are hired, trained and competency assessed including per diem staff.
- Responsible for the oversight of coordination, scheduling and coverage of Nurses and Patient Care Coordination staff ensuring adequate coverage for operating hours.
- Management of departmental budgets including monitoring of overtime and timely completion of timesheets.
- Collaborates with the MA Manager with Orientation and clinical competency education and oversight for Nurses, Medical Assistants and Care Coordinators.
- Coordinate and/or implement new health center programmatic clinical services or service enhancements/modification. Active member with meaningful participation for the Quality Improvement Committee, Operations Committee, Safety Committee, Infection Prevention and Control, Management Team Meeting, PCMH and other committee workgroups/sub-committees including Corporate Compliance Committee activities.
- Oversight of the Immunization Program.
- Works closely with Education department to ensure clinical staff competency requirements are met.
- Oversight of Clinical operations (medical supplies, equipment etc.)
- Oversight of the Covid/Flu Vaccination clinic.
SECONDARY FUNCTIONS:
- This individual will be cross-trained to assist the Nursing/MA staff within the facility in the event of a staff shortage. Although this would be limited to urgent situations or significant shortages, it is the expectation that this individual would be trained in the functions of the various Nurse/MA roles for emergency coverage.
- Must be certified or willing to be certified in CPR as a Trainer for staff and FIT testing, and to assure all employees are up to date in their CPR certifications and annual FIT testing. This will require coordination with the Human Resources and Education departments.
- Provide clear written and oral communication to both Amoskeag Health clients and employees.
- Maintain accurate, timely documentation in the client’s medical record of client contacts, case planning, and client’s plan of care. All documentation in a client’s record to be easily understood by anyone who may need to access the medical record.
- Maintain records of individual staff attendance at in-house education and at outside seminars. Provide yearly summary of education to Senior Management and copy of education record to individual staff.
- Attend and participate in other Amoskeag Health meetings as directed. If absent, responsible for reading minutes of meetings and signing the documentation.
- Present mandatory safety-related in-services as designated by Senior Management.
- Sensitivity to cultural diversity of population being served.
- Other responsibilities as assigned by supervisor.
