A well-established non-profit Visiting Nurse Association (VNA) is seeking an experienced RN to lead their satellite branch in the Springfield, MA area. This program has excellent referral relationships and offers a supportive, patient-centered environment.
Position Summary:
The Home Care Branch Directoris responsible for supervising both clinical services and branch operations. The role includes guiding the team in delivering individualized care, leading case conferences, participating in quality reviews, and ensuring compliance with home health regulations. The Director will work closely with leadership on QAPI reviews and financial performance measures, while also overseeing clinical education and staff development.
Key Responsibilities:
- Oversee clinical operations, including staffing, care coordination, and regulatory compliance.
- Lead case conferences and team meetings, both locally and on the organization’s main campus.
- Ensure OASIS coding, review clinical records, and oversee medical supply orders.
- Provide clinical direction and supervision to field staff, offering education and support as needed.
- Monitor and improve clinical performance through audits, QAPI participation, and performance evaluations.
- Ensure documentation accuracy and regulatory compliance, especially within EMR systems like Home Care Home Base.
- Collaborate with physicians and other healthcare providers to enhance patient care and outcomes.
- Participate in on-call triage and provide direct skilled nursing care in the field as needed.
- Supervise clinical staff, manage orientations, and provide educational programs for both new and existing team members.
- Manage the Telehealth program and ensure accurate tracking of performance data.
Qualifications:
- Graduate of an accredited school of nursing (BSN required, MSN strongly preferred).
- RN license to practice in Massachusetts, with 5+ years of experience in a patient care setting and 3+ years in a managerial role.
- Proven leadership experience in Home Care, Hospice, or Senior Living industries.
- Strong knowledge of OASIS and EMR systems (experience with Home Care Home Base is a plus).
- CPR certification, driver’s license, and reliable transportation required.
- Excellent communication, organizational, and supervisory skills.
