Full time, 8am-4pm. On Call rotation.
Will provide backup coverage when Home Care/Facility Based Team managers are off.
The RN Manager of the Home Health Aide Team is responsible for clinical and administrative oversight of the hospice Home Health Aide Team. Responsible for ensuring the HHA team provides quality care to patients in all settings and maintains compliance with all state and federal regulatory requirements as they relate to aide service offering and the Home Health Aide Training program. Provides float backup coverage for the community-based team managers when needed.
Essential Duties and Responsibilities for the RN Manager of the Home Health Aide Team:
Develops policies & procedures that assure compliance with State and federal regulations, local statutes and other standards.
Provides management oversight and support for the HHAs and HHA team schedulers.
Assists in the development of recruitment and retention strategies with responsibility for recruiting, hiring and terminating of team personnel when necessary.
Assures compliance with the aide training programs and monthly in-services to assure the availability of qualified staff.
Assures that the CHRC, Home Care Registry and HPN are kept up to date and in compliance with DOH requirements.
Serves as a clinical educator for the HHATP once approved by the NYS Department of Health.
Responsible for supervision of HHA team staff, completion of field visits, EMR documentation and coaching and mentoring as appropriate.
Responsible for HHA care plan reviews and updates with backup from the community-based team managers.
Responsible for completion of all placement and sub-aide placement supervisions including the documentation of the supervision to maintain compliance with DOH regulations.
Participates in agency wide continuous quality improvement program, presenting team data at QAPI meetings.
Assists the CCOO in the development of the Annual Budget and the analysis of variances.
Provides on call availability to staff for purposes of scheduling.
Uses leadership position to set positive attainable expectations, objectives and goals for staff.
Assures staff development through goal setting and facilitating education through performance reviews.
Resolves problems expediently, implementing a customer service approach.
Qualifications for the RN Manager of the Home Health Aide Team:
Education/Certification:
Graduate of an accredited school of nursing with current RN licensure in the State of New York.
BSN preferred
Valid Driver's License, free of serious violations, and vehicle required for use when on duty.
Experience:
Minimum of 1-2 yrs of experience as a manager/supervisor in a healthcare environment preferred.
Experience in Hospice & Palliative Care preferred
