Registered Nurse- Hospice

Essential Duties and Responsibilities:
- Provide compassionate direct patient care using established nursing standards and consistent with facility protocols.
- Administer prescribed medications and treatments, meticulously documenting all interventions and responses.
- Carry out detailed assessments of patient conditions, ensuring regular follow-up and accurate documentation.
- Handle the responsibilities of hospice admissions, including the initiation of care plans tailored to the patient's needs.
- Regularly collaborate with an Interdisciplinary Team to perform assessments and update care plans as necessary, ensuring the best outcome for the patient.
- Provide symptom management based on thorough evaluation of patient and family needs.
- Operate and manage technical equipment essential for patient care, such as medication pumps and suction machines.
- Maintain expertise in intravenous access devices and manage the administration of intravenous medications.
- Ensure accurate and timely documentation in the electronic medical records system, complying with legal and ethical standards.
- Coordinate the overall care plan with clinical and support staff, keeping all parties informed of patient progress.
- Partake in hospice-sponsored training and interdisciplinary team meetings to enhance service delivery.
Minimum Position Qualifications:
- Valid and current New York State Registered Nurse (RN) license.
- Minimum of two years of recent clinical nursing experience, with a strong foundational background, particularly in Med-Surg setups.
- Commitment to passing the hospice and palliative care nursing certification (CHPN) exam within three years of employment, if not already certified.
- Must possess proficiency in the use of Microsoft Office, EMR systems, and capability to learn new technical software swiftly.
- Ability to access a vehicle, holding a valid driver's license, and maintaining auto insurance for travel to patient locations.
- Flexible availability, willing to work weekend rotations as required.
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN) is preferred.
- Prior experience in hospice admission and end-of-life care is highly preferred.
