Overview: We are a provider-led organization focused on delivering primary and post-acute care services to senior living communities and residents. Our innovative approach emphasizes prevention and care coordination to enhance the well-being of seniors across our extensive network of facilities.
Summary: The facility-based Advanced Practice Provider (APP) plays a crucial role in delivering direct patient care services within the Post-Acute Care setting.
Essential Duties and Responsibilities:
- Embrace our company's philosophy to provide care that promotes the maintenance or enhancement of each patient's clinical status and quality of life.
- Conduct Annual Wellness Visits and History and Physicals as requested by facilities.
- Perform clinical interventional and episodic direct care visits, including re-evaluations as necessary.
- Evaluate and treat residents for chronic, co-morbid illnesses, providing ongoing care management.
- Review medications, orders, and diagnostic tests, and respond to clinical abnormalities appropriately.
- Coordinate care with collaborating physicians and other disciplines such as Therapists, Specialists, and Social Workers.
- Maintain communication with families and attend family conferences when possible.
- Perform other duties as assigned.
Qualifications:
- Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of operations, with current board certification in your field.
- Minimum of a Bachelors and Masters Degree in Nursing from accredited Colleges or Universities.
- Experience in nursing required, with some experience as a Nurse Practitioner/Provider preferred.
- Strong knowledge of nursing and clinical Nurse Practitioner critical thinking skills applied to aged, chronically ill, and convalescent residents.
- Strong leadership, interpersonal, and problem-solving skills preferred.
- Excellent written and oral communication skills.
- Punctuality, organizational skills, and ability to function within a team.
