Synergy Medical Services is looking for compassionate members to add to our care team. Our caregivers aid our patients in the comforts of their homes by making sure they have the best quality of life possible by promoting and maintaining their independence. Our Home Health Aides provide personal care and related services in the home, under the direction, instruction, and supervision of the Supervising Nurse and as ordered by the physician. They provide hands-on personal care, performing simple procedures as an extension of therapy and/or nursing services, such as assisting in ambulation, exercise, and personal care.
We are looking for caregivers to help service patients in our Region A2 location which includes: Gulf Breeze, Navarre, Pace, Milton, and surrounding areas.
Responsibilities:
- Follow the patient’s plan of care for completion of tasks assigned by the registered nurse or other skilled professionals to provide safe, competent care/service to the patient.
- Help the client maintain good personal hygiene and assist in maintaining a healthful, safe environment.
- Plan and prepare nutritious meals.
- Assist the client with ambulation as approved.
- Encourage the client to become as independent as possible according to the care plan.
- Maintain open communication with the patient, representative (if any), caregivers, and family.
- Provide simple emotional and psychological support to the client and other household members, establishing trust and confidentiality.
- Honor the patient’s rights.
- Observe and report any changes in the client's mental or physical condition or in the home situation to the supervisor.
- Perform routine housekeeping tasks related to a safe and comfortable environment for the client.
- Demonstrate competency with assigned tasks.
- Comply with infection prevention and control policies and procedures.
- Prepare a visit report promptly and incorporate it into the client record weekly.
- Confirm the scheduling of visits weekly to coordinate with other necessary visits by staff members.
- Work with personnel of other community agencies involved in the client's care.
- Attend in-service as required by regulation.
- Confer with the Supervisor about the client’s status and care, participating in the planning of service.
- Promptly report any change in client condition to the Supervisor.
- Work within the Home Aide’s scope of practice, as directed by a Registered Nurse or other skilled professional, and report in writing to the Supervising Nurse.
Qualifications:
- High School Diploma or equivalent
- CPR Certification
- Level 2 Background Check
- Must be able to pass Criminal Background Check, National Sex Offender Registry clearance, and OIG clearance.
- Possess and maintain a valid Driver’s License and Automobile Insurance
- Reliable Transportation
- Able to pass 75-hour FL HHA Certification Course – or - proof of certificate if already completed.
- 1 Year of previous experience providing personal/direct patient care
