DUTIES and RESPONSIBILITIES:
- Provides excellent patient care that is focused on wellness, prevention of illness, and acute problem and chronic disease management.
- Identifies patient normal and abnormal findings through assessment of the physical, psychosocial, and developmental status of individuals and families, which is performed through independent and interdependent actions including health history, physical exam, laboratory screening, diagnostic tests, referrals, and collaboration with other medical providers to develop appropriate and quality diagnostic treatment.
- Teaches, counsels, and provides emotional support, as it is an integral part of the provider’s role in providing the continuum of care for HealthFirst’s Clients.
- Assumes responsibility of ongoing health maintenance by initiating pro-active health care needs of patients to carry out their prescribed medical regime and utilize resources to maintain an optimal level of health and wellness.
- Assists in the development and implementation of disease management programs in terms of outcomes and goals for HealthFirst’s clients.
- Participates in Quality Assurance and Performance Improvement activities.
- Maintains patient care and practice in accordance with federal, state, and agency guidelines.
- Establishes, implements, and evaluates research and standards of nursing practice to assure quality care.
Requirements
EDUCATION/EXPERIENCE: Licensed as an Advanced Registered Nurse Practitioner or Physician Assistant in the State of New Hampshire. Graduate of master’s degree level nurse practitioner or physician assistant program.
KNOWLEDGE, SKILLS, and ABILITIES:
- Excellent written and verbal communication skills
- Ability to interact within a team setting
- Demonstrate professional behavior and appearance
- Displays a courteous and supportive manner with staff as well as a competent, confidential, and compassionate manner with patients
- Ability to assist clinical staff in interpretation of medical charts, monitors, and other pertinent patient medical data
- Computer skills and documentation in electronic medical records is necessary
- Collaborates with members of other disciplines, community agencies, school personnel, and hospital staff in meetings the needs of the community
- Acts as a consultant, resource, and support for the clinical staff
